Which rate applies?

It depends on the country and the product category: most EU states use a standard rate between 19 and 25%, with reduced rates for food, books and medicines.

How do I correctly remove VAT from a total?

Divide the total by one plus the rate. At 20%, divide by 1.2; the VAT is the difference between the total and the result.

Why is it off by a cent from my invoice?

Rounding. Invoices round per line or per total, and the two methods can differ by a cent on large amounts.
